Donut Hole or Coverage Gap: Once your total drug costs (including what you and your plan have paid) reach a certain threshold, you enter the Donut Hole or coverage gap. In this phase, you become responsible for a higher percentage of your medication costs.
Catastrophic Coverage Phase: Once you reach a certain out-of-pocket spending limit, you exit the Donut Hole and enter the catastrophic coverage phase. During this phase, your drug costs significantly decrease, and you only pay a small copayment or coinsurance for your medications.

While in the Donut Hole, you are responsible for a larger share of your prescription drug costs. In 2023, you will pay no more than 25% of the cost for both brand-name and generic drugs while in the coverage gap. This percentage will continue to decrease over the years until it reaches 25% for brand-name and generic drugs in 2025.
It's important to note that the portion you pay out-of-pocket, including the deductible, coinsurance, and copayments, as well as any discounts provided by drug manufacturers, count towards your total out-of-pocket spending and determine when you exit the Donut Hole.
